What To Expect

On your first visit, we will begin your permanent record with our office.

1. You will need to fill out a registration and medical history form. Please remember to bring your insurance benefit identification cards.
2. We will also be taking a medical history. Please bring a list of all medications and any eye drops you are currently using.
3. Be sure to bring your glasses even if they are broken so we can get a reading on the lenses.
4. If you normally wear contact lenses, be sure to wear them unless they are causing you pain.

At every visit, please check in with the receptionist when you arrive and inform her of any changes in your registration form such as address, telephone number, insurance information or occupation change.

A technician will call you from the reception room to prepare you for the examination.

Please allow 1 ½ hours for your first visit to our office.

If special tests or procedures are necessary, your visit may be longer. We try our best to stay on schedule, although sometimes emergencies arise. We will notify you if we have a long delay as we know your time is valuable.
