Zafar A. Sheik, MD

Zafar A. Sheik, MD, a native of Cleveland, joined Warren Ophthalmology Associates in September of 2001 and purchased the practice in July of 2003.

He is currently the Chief of Ophthalmology at St. Joseph’s Medical Center in Warren. Dr. Sheik came to the Warren area after completing a fellowship in Corneal and Refractive Surgery at Allegheny General Hospital and the University of Pittsburgh.

He is one of only two fellowship trained corneal surgeons practicing in the Mahoning Valley. He is a board certified fellow of the American Academy of Ophthalmology as well as a member of the American Society of Cataract and Refractive Surgery.

Dr. Sheik specializes in small incision cataract surgery, refractive surgery, and corneal transplants.

He graduated from the Medical College of Ohio and completed his ophthalmology residency at the University of Maryland. He received his bachelor’s degree in Electrical Engineering from Cornell University.

Dr. Sheik currently lives with his wife, and two sons, in Canfield.

Frank A. D’Apolito, OD

Frank A. D’Apolito, OD

Frank A. D’Apolito, OD is a life long resident of Youngstown-Warren area. He joined Warren Ophthalmology Associates in 1982.

Dr. D’Apolito received his BS in Biology from Youngstown State University. He received his Doctor of Optometry from the Illinois College of Optometry where he was recognized as the outstanding clinician of the year and presented with the Lawrence P. Feigenbaum Award. He concentrated his studies on contact lenses and eye disease.

Dr. D’Apolito completed his externship at Murrough’s Eye Clinic in Santa Barbara, California. He also interned with Jessen Wesley Inc.,a private practice in Chicago dedicated to contact lenses.

Dr. D’Apolito continues to perform clinical research of contact lenses. He has lectured for Ciba Vision and Vistakon Contact Lens Companies.

Dr. D’Apolito is a past president of Ohio Vision Foundation and the Northeastern Ohio Optometric Association. He has also served on the boards of many community organizations.

Dr. D’Apolito and his wife, Valerie, have two boys.

Carla Darak-Bolino, OD

Carla Darak-Bolino, OD

Carla Darak-Bolino, OD has been an associate at Warren Ophthalmology since 2003. She is a graduate of the Illinois College of Optometry, where she received the Wesley Jessen Contact Lens Achievement Award in 1997 for her excellence in performing contact lens examinations and fitting.

After practicing in the city of Chicago for over five years, the Warren John F. Kennedy and Kent State University graduate brought her experience back to the Warren area in 2002. Her dedication has extended beyond her work in the office, as she has volunteered her time with Chicago elementary school students, the Special Olympics Word Games in Toronto, and with the citizens of Haiti.

Dr. Darak-Bolino, husband Paul, and two sons reside in Warren.
